Hyperlink in Form?

how i can create a hyperlink in form i need a similar function like this page:

Hi Osa,

Which function on that page are you trying to emulate?
The English / Espanol links?

For that I would use the "Display" form type, and set the URL to go to on the "OnClick" settings for that field.

is for emulate Forgot your user name? and Forgot your password? in same positions.

Ok, so use a Display field as described, and place it to the right of the entry fields.
