File Uploading not working in NT 11.04 Disconnected app

After many hours (frustrating ones!) of trying to use FileUpload, I have come to the conclusion it is not working properly.  Usually this means I have configured something incorrectly.  But, I have duplicated the settings I used in a NT10 web app that worked as expected.  The issue is that after successfully uploading an image to the uploads folder, the name of the uploaded file has not been put into the tps file (server) and local store (IndexDB).  See the attached image.  This means the image control has nothing to grab.  Can you look at the last example app I sent and see if it works for you?

After about three weeks of fighting with this app, I'm about ready to give up.

Perhaps it might help to see how I have things configured.  I have attached images of the template entries I have set.  All other tabs on the Image control and FileUpload control, not seen here, have no entries made.

Also, specifically on the Image control, Display tab, I have questions about the first three fields: 

1.  Display:  Choices are Text, SessionValue, HostValue, File Tag <!--Netf
     Can you explain these options and what should typically be my choice here?
2.  not labeled
     This is not labeled, what is this used for? 
3.  Picture:  is this to set a typical picture for a field?  If so, what field?  This is an Image control and should just be showing the uploaded image file who's name should be stored in the field CC:Coin_Image_Name_Obv
