NT12 Drop down menu

Hi,
1-in one of the main items, in the submenu items shows an icon like there where more subitems and its not.
Why is this happening?

2-How to make a sub submenu? is there any example?

I think its a bug.
Problem is when you, in the main item has a 'Procedure' in the Click tab, if you delete the 'Procedure' field and write it in the 'URL' field the problem is gone.
