Lets Encrypt

Hi, I have an NT server running in a VPS
It dns is www.docrotanyplace.com
its compiled with NT9 and running in port 80
all ok

Now I want to replace it with a NT10 compiled version and use the Lets Encrypt certificates and Im confused
How do I set the NT Settings for a certificate?
1)
CA Account :  DoctorAnyPlace
and
Domains: www.doctoranyplace.com ?

or I need to create a new subdomain like

2)
CA Account :  DoctorAnyPlace
and
Domains: secure.doctoranyplace.com ?

And if I need to use 2) users that type www.doctoranyplace.com will go throught port 80 and secure.doctoranyplace.com with ssl port?
