Probably the key reasons for going to NetTalk 7 are "under the hood". Most of the improvements I like the best are things you'll never notice directly. Things are just a "little smoother" across the board. The dynamic compression (while almost completely invisible to you) is one of my favorite features. It reduces packet size, which in turn means improved performance. The Multi-Site host logging is also improved a lot so that helps with debugging etc. Auto-checking cached files also up there.
On the interface front the new File Uploader is probably the most visible change - but there are also spin buttons, media controls, and so on. This goes beyond the actual visual control - it also gives you more control over the size of incoming files, and also gives you more opportunity to validate them before they are saved.
Other little touches as well like popups from links on browses and forms, the new Redactor HTML editor, updates to jQuery and so on all contribute.
At the end of the day it's up to you to decide if you want to upgrade or not, or when in the cycle you want to do that. It's an on-going project so there's new stuff happening all the time. Whenever you're ready is the right time for you.
